In the one study that most people point to they gave metformin to people who met the criteria for being overweight based on their body mass index (BMI). They also gave a placebo to the next group of people who met the same criteria and compared the two. I don't want to get into the specifics of the study but the bottom line is the group that got metformin did lose weight. The group that got the placebo gained weight. BUT - they didn't control for any other factors like changes in diet and exercise. Pretty much anything you do for someone in that situation is going to help them. Improving insulin sensitivity is a great first step. So for these folks metformin will definitely help. The problem I have is that too many people are looking for a pill to help them lose weight. You can essentially do the same thing metformin is doing (improving insulin sensitivity) by: 1) Eating in a caloric deficit of 25% 2) Resistance training 3) Cutting out processed carbs Simple? Yes.
