One potential side effect of testosterone replacement therapy (TRT) is azoospermia, the absence of sperm in semen, leading to infertility.  This study indicates this occurs in roughly 40% of men undergoing TRT. PMID: 35783920 The good news?  This is typically reversible! Studies show that sperm production resumes in the vast majority of men within 24 months of stopping TRT and much earlier for many others.
